Summit Care Partners (SCP) is a Portland-based Health-Related Social Needs (HRSN) provider dedicated to improving health outcomes by addressing housing instability and the social conditions that impact well-being. We partner with Oregon Health Plan members to deliver Outreach & Engagement and Tenancy Support services that help individuals obtain, maintain, and stabilize housing.

Our HRSN Services
Outreach & Engagement
Core O&E Activities at SCP

Community-based outreach in shelters, clinics, community organizations, and encampment-adjacent settings

Education on HRSN benefits, eligibility, and referral pathways

Assisting members with initial HRSN referrals and documentation readiness
-
Connecting members to CCOs, housing providers, legal aid, behavioral health, and primary care
-
Follow-up engagement to ensure successful linkage to services
Service Standards

Services initiated within 30 days of referral

Trauma-informed, culturally responsive delivery

Language access and ADA accommodations provided

O&E services are non-duplicative and do not overlap with tenancy support
Tenancy Support
Core Tenancy Support Activities at SCP

Housing navigation and housing plan development

Landlord communication and advocacy

Assistance understanding lease terms and tenant rights

Eviction prevention support and referrals to legal services

Coordination with property managers and housing authorities

Ongoing housing stability check-ins
Service Standards
-
Member must be currently renting housing with one of the following:
-
A lease signed by both the landlord and Member; or
-
A completed HRSN Verification of Landlord/Tenant Relationship and Rent Owed form signed by both the landlord and Member; or

A written agreement signed by both the landlord (or equivalent entity) and Member with the components outlined in the HRSN Rent and Utility Costs service description
