We are committed to delivering person centered, trauma informed services that empower individuals to achieve lasting housing stability and overall well-being.

About HRSN

In Oregon, HRSN refers to services funded through Medicaid (Oregon Health Plan) that address non-medical needs directly impacting a person’s health. The idea is simple: health doesn’t start in a clinic. It starts in stable housing, food access, safety, and support.

Eligible OHP members may receive time-limited, medically appropriate services from SCP in the following domains:

 

  • Outreach & Engagement (O&E)

  • Housing Supports (Specifically Tenancy Supports)

Outreach & Engagement

Outreach & Engagement services support eligible OHP members in understanding, accessing, and remaining connected to HRSN benefits and other health-related resources. O&E focuses on engagement, education, navigation, and warm handoffs.

Tenancy Support

Tenancy Supports are housing stabilization services that help OHP members obtain, maintain, or avoid losing housing. These services are non-financial and focus on skill-building, advocacy, and coordination.

We offer a range of referral services to help underprivileged adults navigate challenges and access essential resources. We become your partner through person centered planning. From career counseling to financial assistance, we are committed to empowering clinical and social risk adults in our community.
